Hi Eli,
I don't think you can do this with Questionnaire (it's a survey tool) but you can with Hot Potatoes if you insert a table in JCloze. Here are some examples :
- Carré magique (magic square)
Hot Potatoes is a native module in Moodle 1.9 but it has to be activated by the admin; in Moodle 2 it has to downloaded and installed as a plugin.

You could create a quiz question like the attached using the cloze format.
