Sorry to post an old problem here. It is similar to this one, reported years ago:

When a user doesn't respond to a required field in the questionnaire, the form gets reposted, causing a backslash il front of every apostrophe. This of course occurs in a text field.
I was able to fix the bug in the Journal module with "stripslashes".
Is there anyway I could do the same thing in the Questionnaire module?

Which version of Moodle are you using?
Which type of question are you seing the apostrophe problem in?
If the user has not responded, how can backslashes appear in front of non-existent apostrophes?

I'm using 1.9.12.

Thanks for the report. I can confirm the bug. It has been fixed for moodle 2 but not for 1.9. I will investigate and report here.
