Hi Roger,

I'm sorry but the layout in the feedback for 1.9 is not optimal . It is a simple table and can not be fitted in the way you want.
In moodle 2 you can manipulate the layout for these questions a bit better. But only by manipulating the theme-settings and not by settings inside the feedback-module.
The "Not selected" option-field is optional since moodle 2.
