hi Joseph - admin settings >manage activities shows version of installed Questionnaire module is 2008060403 - guess this is an older version for 1.8x ?
The site is hosted by an external provider - I dont have access to upgrade module files - so if you can please confirm the current version of Questionnaire that will be compatible with Moodle 1.9.9 I will log a job with them to upgrade Questionnaire files.
