I am creating a question that is not a quiz question. It is more of a survey questions, but i am using a quiz in order to keep track. It says:

Yes, I am interested in pursuing genetic risk assessment.

No, I am not interested in pursuing genetic risk assessment at this time.

I want the student to be able to check off a box that is next to each sentence. Can I do this in quiz mode?

You can do this with a multiple choice question with single answer.

I do not know what you will do with the answers of the students.

I prefer for that the feedback modul or the questionnaire module. The second one is not a standard module in 1.9 but works well.

Thanks for your reply. I need to get an automatic email with the answer so that I know who answered what (I know how to set that up). So the multiple choice option might work. what do you mean when you say that you prefer the feedback module or the questionnaire module? Do you mean instead of using a quiz? I don't either of those offered as activities.

I tried making it a "Choice" instead of a "quiz". My questions is will I be able to get emailed with the automatic notification of results like I can do with a quiz?

AFAIK any email notification for an assessed activity (at least a quiz attempt and an assignment submission) only advises the teacher that an attempt has been done with a link to the activity. The email does not contain any data from the attempt/submission.
I like the choice activity for this sort of 'poll' however if you need email confirmation when someone has answered the quiz will work just as well.
Jürg mentions the Feedback and the Questionnaire modules, these need to be downloaded from the Modules and Plugins page and added to your site as they are not standard modules.

Actually, the e-mail notification for Questionnaire does contain the whole response data for each submission.

Where can I find this questionnaire that you are talking about? Maybe it is in a more recent version of Moodle. Would it be listed as an activity? I would love to have not only the attempt emailed to me, but also the answer. I think I have Moodle 1.9.

The questionnaire is a 3rd party module, not part of regular Moodle distribution. You can find it, with lots of other modules, in the Modules and plugins database. If you are not proficient with moodle OR computers, you'd better ask your admin to do it for you.
