Using Moodle 1.9.2+. Our admin went to edit a copied quiz. She saw a message saying that "No questions had been added yet." However, she could still preview the quiz and see questions. We then logged in as a test learner and were able to attempt the quiz after ignoring the message that "No questions had been added..."
There is an activity log record for the test learner showing a quiz attempt. When we review the quiz attempt we see all of the questions. However, in the Edit mode for the quiz we don't see any questions as having been added.
