I would suggest you create this type of scenario in a Quiz.

You have a lot of flexibility in the Scales (create a Custom Scale to reflect your 5 point scenario) and Grading (different points allocated to individual questions, or groups of questions (categories), and the overall score boundaries can be used for the end result.
