Good Evening, I upgraded the new moodle and had that working this morning. I then tried to add the questionnaire module and am now getting errors: Fatal error: Call to undefined function: get_string() in /home/dpaweb/training/moodle/mod/resource/lib.php on line 13

Fatal error: Call to undefined function: ini_get_bool() in /home/dpaweb/training/moodle/admin/index.php on line 20

Have you accidentally overwritten some of your core Moodle files?  Looks like it...

Config file, config file, config file.... not sure what keeps happening but when I just use the old one and go with it, everything works fine. I must be leaving something out each time. Thanks for a GREAT product!
