Are you using 5.0.1? This looks like a bug that was fixed (but I don't have the number to hand.)

This seems unlikely. You can verify what happened by looking in the logs for this quiz for this user. My guess is that at some point, this sudent started an attempt. (If you review the attempt, you can see when it was started.)
The other way to fix this scenario, other than overriding to allow a second attempt, which is a good option, is to go into the quiz reports, and then you can delete the unwanted first attempt by this student.

The second bullet point here is not correct.
After the quiz has been attempted, you can go and define groups and grouping, and then set the quiz to use those to display the results, if that is what you want.
