I think this did cause a problem for the upgrade from 4.5 -> 5.0 so the upgrade code was enhanced to automatically solve this as part of the upgrade: MDL-84305.

I don't understand Rick's suggestion. From his message, this is correct:
"Questions previously created in the course category context are saved in a course shared question bank in a new course named 'Shared teaching resources for category: [Category name]'."
That is what the code does. Has anyone seen any different to that?
