Glad that you could solve your problem. But that doesn't mean it will solve Alžbeta's problem https://moodle.org/mod/forum/discuss.php?d=468310#p1899225. You are talking about the question type "random" which should have been deleted a long time ago, I think in the transition from 4.1 to 4.2.
 
Or in other words, the exploding question banks have (had) multiple causes. From what I read, at least one cause is still in 4.5. Only 5.1 is fully fixed.

Well, I would upgrade to the 4.5 LTS current, which is 4.5.11 from 20 April 2026. I don't know whether the duplication problems were fixed, but because of the numerous security bug fixes (since June 2025).

I can confirm first-hand what Emma said: Moodle 4 is known for duplicating questions exponentially. See also the numerous discussions in this forum.
 
But what surprises me is, that the problem still exists in Moodle 4.5. What is the exact release of your Moodle 4.5.x?

Maybe our feelings are ultimately that of these models, or, if we want to think positive, we can argue that the models inherited their "feelings" from us! Whatever, I value _your_ feelings more than a collective conscience (con-science?) of humanity, as sucked by these agents.

I know, I know, in an argument over feelings, there are no winners - exactly that makes the discussion exciting. But one thing is factual - for me at least - the centralization, the world-wide code base in one place, and in the vaults of a US BIG TECH of all places.
