which setting do you have?
If you set it to NONE or MINIMAL, you should no longer see these Notice messages which are only aimed at developers and do not prevent Moodle from working.
Can you check?

Hi Mike,
First thing, Moodle 1.5.4 is really very old stuff now. Can't you upgrade at least to 1.6?
Anyway, it is quite impossible to tell what is happening to your questionnaire editing, since, as you say yourself, behavior seems to be completely erratic. As Admin, can you try turning debug mode on to see whether you have any specific error messages displayed when things go wrong? When you say that "the questionnaire disappears" from teachers websites, what do you mean?
Do you have a standard Moodle installation or heavily customized? Do you have 3rd-party modules installes besides Questionnaire?

When you say "things seem to be working", can you actually do a Cross Analysis and a Cross Tabulation such as displayed on the questionnaire online Help screens?

When I apply your modification to locallib.php file, line 2114, I can now access the CrossTabulate page, but what I can see is not what is shown in the online help. I can click some radio buttons but nothing happens.

If you can do something, please post here how you proceed and some screen shots showing what happens.

You can use any tool which enables you to search all files in any folder containing a given character string. Unfortunately, Windows XP's own Search tool is very bad. I personally use Windows Commander, I've used it for years.

There have been quite a few reports about the Cross Tabulate function not working in the Questionnaire module (from Moodle 1.6.4+ to 1.8). Apparently this function is no longer maintained. I have never been able to get it to work on my local installation of Moodle.
