You should have said so earlier... Anyway, I do not see the problem. The questionnaire templates are created like ordinary templates, by choosing Add an activity -> Questionnaire in a course on your moodle site. Simply locate those you want to delete and delete them. Of course, you'll have to locate the course where they have been originally created.

1- First problem
In your previous post you wrote "I still can find at row 1918 <td colspan=2 align="left">" I answered that there was no validation problem with that code. Now you write that the problem is with <td colspan=2 align="left"><b>. Do you mean that it is the <b> tag which is not correct? In fact, although it is recommended that the <b> tag should be replaced with the (semantic) <strong > tag, the <b> tag is still allowed in XHTML 1.x Strict. What more can I say? We are not (yet) aiming at XHTML 2.0 I think.
In any case, it does not matter because - as you have found out, "the function mkresrank is never executed". There are still a number of obsolete or redundant code lines in the Questionnaire module, because that module has beeen modified a lot over the years, and I have concentrated my efforts for XHTML 1.x Strict validation on the code which really gets executed. I checked all pages with http://validator.w3.org/check.
2. Second problem
You are right about the "Your responses - view x" page (myreport.php). There was an extra </div> tag in line 86, which I have removed. I have committed the change to contrib (for 1.8). Thanks for pointing out the bug.
