Hi Mike,

Please note that the qu_orangechoc theme was meant to be used in conjunction with my own orangechoc moodle theme. It is not guaranteed to look satisfactory when used with other Moodle Themes, either standard or customised. You do not say which Moodle theme you are using in your course. What about using the Questionnaire default theme?

There should be no difference between what you call the "live" mode and the preview mode, so I do not quite understand what your problem is.

Anyway, so that I can see what is wrong I do need more information. Could you please save your questionnaire (in your moodle course, go to the Admin / export link, de-select everthing except your Questionnaire (without data) and post here or send to my e-mail address (see in my Moodle profile).

Or, at least, you could post here screenshots so we can see exactly what things look like on your Moodle.

Hi Neil,
Are you sure there are groups in your Moodle course?
When you go to the View All Responses tab, do you see the All participants, etc. dropdown list as shown in attached screen capture?

Hi Neil,
Which version of moodle and Questionnaire are you using? In version 1.9 it is certainly possible to export group names to the exported data text file. Of course your questionnaire settings must be set as Groups separate.

True, but OP wrote "Users are able to take the quiz, but for some reason the quiz results don't seem to be recorded,..." For users to be able to take a quiz they must be enrolled in a course, no?

Well, your screenshot clearly shows that there are "No students enrolled in this course yet".
