It would, indeed, be useful to say different things about yourself in different courses.
Especially on multilingual moodle sites, it would be very useful for users to be able to give a description of themselves in the current course's language!

I reported this bug as MDL-23575 on 29/Jul/2010. You are welcome to vote for the bug report to hopefully get it fixed by Moodle HQ programers any time soon.
In the meantime there is a simple workaround. Insert the image you need inside the Question text field, then copy-paste to the Answer field.

The "secret" to insert smileys (or indeed any image) in the "choices" is to first insert them in the question text (in the HTML editor), then switch to HTLM code, copy the code and paste it into the choices box.
I am attaching a sample questionnnaire with the questions I created. Of course, if your restore that backup to your moodle site, you'll have to replace the "local", absolute address of the smileys with their address on your site.
