Hi Barrak,
You have downloaded the wrong version (for moodle 1.9). You need to download the version for Moodle 2.0, which installs OK.

Hi Lee
A 50-question questionnaire with 300 respondents, that's 1,500 questions altogether, and probably putting the stress on your server. You should turn debugging mode ON (briefly if that's a production server) and check if you get a PHP error report. Very likely a time-out error. On your PHP server you should consider increasing the time allowed to PHP scripts execution time.

katja: "I found the problem: all MC answers (correct and incorrect) must be set to 'jump to next page."
This is not the way MCQ questions are supposed to work in the Lesson module!
Correct answers must be set to "jump to next page" and incorrect ones to "jump to this page".
You must have "played" with the other Lesson settings somewhere (there are so many parameters...).

Kara,
Which version of Moodle are you using: 1.9 or 2.0?

Hi Paul,
Thanks for the detailed explanation. I'm afraid yours is a "special" request, due to the special way you are using the Questionnaire module. I hope you will find the relevant solution for your special use.
