Yes, I tried to just copy the relevant parts from the dev build but apparently didn't copy everything.
The attached file should include the most essential part for the fix and should allow the notification to go through. The sent notification is minimal. In the next release it contains a link to the added/updated entry (see illustration).
For testing purposes note that designated recipients need to have the respective capabilities.
Yes, something got broken there. It is already fixed for the next release. The attached zip contains 3 files that should fix it on your installation. mod_class.php and locallib.php go into the main dataform directory and dataform.php in the lang directory. Please let me know if it solved the problem.
Yes. It is integrated into the standard build and you just need to updgrade Moodle to the latest stable for enjoying the fix.

Well, in the Dataform you can arrange the fields in any order you wish and these settings are saved and stored so you can use the same export settings in subsequent exports and you can also have several export templates according to need. (See illustration) But then again, that's not the Database so may not be of much help to you.
It shouldn't be too difficult to add ordering to the export form of the Database module if someone decides to take up the enhancement.
