Thanks again. With the help of John Isner I have managed to get to the bottom of it. Users were being treated as "Guest" rather than Authenticated Users. I needed to make a change to Site administration -> Users -> Permissions -> User policies. The "Default role for all users" should have been Authenticated user, but it was set to Guest. Certainly the setting wasn't changed by anyone since the upgrade so this sounds like it might be a bug as after upgrade it defaulted to "guest" rather than "authenticated user". I've submitted a bug to be investigated.
I will now investigate our other problem of teachers not being able to use the "loginas" option for their courses and post back if necessary.
