Hello Joseph,
Thank you for the update. The problem is solved.
Now I have another problem, is that the buttons inside the questionnaire report are not working, "View By Response", "All your responses" those are inside the myreport "Your Results" tab.
Also another bug, when viewing all responses and trying to delete them. I get a confirmation page either to delete "Yes" or "no". Click on "Yes" everything works fine, clicking on "no" I am stuck and not redirected to the course main page.

Yeah Joseph, I have updated the three files you told me about.
I have tested on your test site, and it is working fine.
How can we troubleshoot from here then?
As I mentioned in my previous posting, half of the problem is solved, but I am stuck inside the report.
Any suggestion?
