I've updated the CVS to fix a bug in the questionnaire module, I've also included the file here for others.
The existing module would mark a questionnaire as completed even if all required fields had not been completed. This resulted in an inconsistency between Moodle and phpESP, where Moodle thought the questionnaire was complete while phpESP did not.
I also added Moodle logging of submittals.
Grab it from CVS, or copy the attached files into your 'mod/questionnaire' directory.
