You could get this to work with the core Feedback activity, as there's a setting that will enable sending notifications to staff members with a teacher role. The Feedback activity is basically a questionnaire with a number of question types, such as short answer and multiple choice. There's also an essay type question that you could use.
Using the restrict access and activity completion settings, you can provide access conditions for subsequent activities, such as a discussion board or Diary activity, as AL suggested.
