This is my first time using the Questionnaire plug in and I am trying to figure out
if I am doing something wrong or if there is a bug
Both the Name and Description are duplicating.
I created a very simply example attached.

Thanks for reporting this bug. There is already a bug report at https://github.com/PoetOS/moodle-mod_questionnaire/issues/420 and a proposed fix (not yet released).

This has already been fixed in the MOODLE_400_STABLE branch.

I have Moodle version 4.0.5 and I downloaded the questionnaire version 4.0, I still get the error.

I'm not sure where you got the version you are calling 4.0? If you mean you downloaded the latest version from the plugins database, that would be 3.11.1.
If you need the fixed version now, you have to download from Github using the link above. The specific issue was fixed in this commit.
