Ok, strange one this as these docs are not accurate - https://docs.moodle.org/dev/Themes_overview#Files_and_folders - or that there is an old way that still works using the 'pix/mod/modname' folder e.g. -> https://github.com/gjb2048/moodle-theme_foundation/tree/master/pix/mod/quiz.
