We have managed to inadvertently create several copies of the same questionnaire template by "duplicating" the questionnaire in a course.

Is there a way to delete the extra templates, which are nothing more than than copies of a template with the same name?  

We have already deleted the questionnaires themselves in the course that were duplicated, but when creating  a new questionnaire in a course and choosing clicking on "Content Options", every template ever created/duplicated still appears in the list of radio buttons.  We have cleared the cache, etc.  The database likewise contains an entry for every template as well as duplicate entries for all questions.  Right now the only way we can see to remove the templates from the list is to write a database query to delete every duplicate template and every question belonging to that template.  There are no questionnaires actually filled in, so we are not worried about any feedback data, just keeping the questions that have been created for several templates and only having one copy of each template.

Deleting any questionnaire from the course it belongs to should be all you need to do. Are you perhaps using the recycle bin? If you are, it will remain the database tables until the recycle bin is emptied.
