Questionnaire (plugin)

Hi alls!,
I've founded few presistant bugs in questionnaire:

1) when you check date at the fist page, when you finish seting up the questionnaire at the confirmatin page, the date are unchecked! (same problem is you don't check date box, they'll be checked)

2) when you're making question: when you clck "add a ligne" for one more response,  the result is the same as if you click on "new question".

3) when you start creatring a new questionnaire, you fill all the pages,... arriving at the final confirmation page you forget to click the final "save" button.
Result your questionnaire 'll be viewable on public or private questionnaire bank but no way to delete it!
