The only way of doing that type of layout with a book would be to modify the book code and add that capability yourself, pay someone else to modify the code, or put in a tracker request and hope the Moodle developers do the modification for a future release.
An alternative would be to create your desired layout in a SCORM, which is what I used to do whenever I wanted sub pages. I used the free SCORM editor eXe.
