I found the answer, although I do ot understand how it works. The page was being linked to from the previous page. The link was missing '&thankyou=1' at the end of the address, eg

'http://www.kneeguru.co.uk/courses/mod/wiki/view.php?id=191&userid=0&groupid=0&wikipage=WHY+NOT+EVERY+MENISCUS+TEAR+CAN+BE+REPAIRED&thankyou=1

When I added this, the tabs reappeared.
