You'll have to add at least one blank line after each question. After I did that your file worked fine for me. I'll make sure to document that more clearly. In addition, I also just noticed a bug where the last question was ignored if not followed by a blank line. This is fixed now in CVS, so even your one-line file works.
Loren, you shouldn't be getting a blank page, at the very least you should be getting an error page. Are you sure you have the very latest nightly? And all of it? Some files have been moving, and a blank screen usually indicates a file is not in the spot that Moodle is expecting.
