Cant see add..questionaire in moodle course

hello

i just installed questionaire, i think 4.1
I have gone to the admin page done everything, created survey, activated it. But then when i go to my moodle course and click on 'add' there is no questionaire option there, please help, its urgent for my class.

It sounds like the module hasn't been added. When you went to the admin page, did it show that it added the questionnaire module? Try going to the configuration section of your admin page and check which modules have been installed.
