Can Moodle do this?

I need for students to complete a task sheet, indicating that they have done what is required, then have the teacher initial (or indicate) that the student has indeed completed the task so that the boss of the teacher knows. 

For example:
                                                 Task
                            Student Initial      Teacher Initial

Student Name               ABC                     DEF

Can Moodle do something like this?

Sounds like a job for assignment module.
Set it up as an online assignment, perhaps with emails to the teacher when the student submits.
The teacher's boss will need to be a member of the course or an administrator to see the results.
You could go further and have the students submit brief comments on how the task went and then the teacher can give feedback.
It should be fairly straightforward to pull a table such as that you suggest above out of the underlying database, but the assignment grades table itself might provide all that you need.

Hi Angela,

If you are trying to replicate a form--one with various blanks (data fields) that students fill in to show their progress on a task--then this might be an ideal application for the Database Module (formerly called the Form Module). This module is in testing now and will be released in version 1.6 (sometime before the end of the year). I am not sure if teachers and administrators can "initial" the form, but it sounds possible.  There is also a Task Module in the works by Jeff Graham which may do what you want.  For now, try Miles suggestion with the Assignment Module. smile
