Are there any calls to the allow_call_time_pass_reference function in any of your other 3rd party modules?

I doubt that the Questionnaire module is the culprit here. There is no call to function allow_call_time_pass_reference in any of its files. The only module which calls this function is the chat module. Maybe you have customized your Moodle, or added 3rd party modules?

When you are logged in as dummy student, do you take and submit the questionnaire and click on the Return button at the end?
If you take the questionnaire as a teacher or admin and submit it, do you still get "no responses"?
If you look into your Moodle mysql database with phpMyAdmin, Table: mdl_questionnaire_attempts, in the timemodified column, do you see any timestamp which you recognize as related to your most recent attempt? or none at all?
Have you checked your Questionnaire settings? Is Questionnaire Type set as private, public or template?
