By default, only teachers have moodle/course:manageactivities ("Manage activities") = Allow, which lets them turn editing on in a course to add and delete activities, blocks, and resources. You probably don't want to give this ability to students. So the teacher will create each activity, block, or resource, then give students the appropriate permissions within it. For example, a teacher can create a quiz, then give one or more students the the appropriate quiz and question bank permissions needed to create questions and add them to the quiz. By "appropriate permission" I mean that different permissions will be needed in each context. Start by defining the requirements for each activity, block, or resource. From that, you get permissions in different contexts, and from that you can come up with a design using one or more roles, overrides, etc.

Has it been discussed anywhere? You would have to search the documentation and forums for each activity type. For example, here is a discussion about student-created questions and quizzes.
