Questionnaire (plugin)

I have created a questionnaire template so that every department in my college can create a questionnaire in their own subject based on my questionnaire. When I was trying out the template I completed the questionnaire just to test it out. Now all the 'copies' based on my template have this response added to them and I can't seem to delete it from the template! If all that makes sense...any ideas? Thanks in advance.
