Questionnaire problems (click on the link and a blank page appears)

Hi in regards to questionnaires in Moodle our College is having trouble accessing questionnaires or even creating a questionnaire. When a person clicks on a link for one it returns a blank page. Has this happened to anyone else? Our website was hosted on a Linux server on the campus and now it hosted off campus, but the rest of Moodle works fine other than the questionnaires. Any ideas? Thanks.

Same problem here - we can edit the questions but just get a blank page when clicking on the link to the completed questionnaire.

I, too, am having a version of this problem. Questionnaires can be created and filled out by course participants, but the results cannot be viewed. When an instructor clicks on the "View all responses" button or a student clicks the "View my response" button, they get only a blank page.

I will be delving into the code to try to fix this problem, but it is interesting to know that other people are experiencing this as well.

I am running Moodle version 1.8.3. What version are you running?
