The old document is still cached somewhere (not necessarily in your own browser, could be anywhere between your server and your browser). The document has a lifetime of 24 hours by default but you can change that by setting $CFG->filelifetime to the number of seconds somewhere in your config.php file.
The only way to make sure that the new file is available immediately is to give it a different name.
