We had a problem with a quiz. At first, there were 10 random questions, pulled from 2 categories. About 5 students took the quiz successfully, and the last one posted a result at about 9:00 a.m. on Thursday morning. Then the quiz died. All students after that time could load a partial quiz--sometimes 5 questions, sometimes 7, sometimes only 2. In all cases, the entire page did not load and the submit quiz buttons at the bottom were not displayed on the screen.
I took a look at the instructor's quizbank, and noticed that she had 3 identically named categories. We made each category name unique, but I don't think that helped any.
Some of the students reported this error at the top of a screen:

I checked the question ids (by looking in Moodle, not the database), and I could never find these two. I think at some point they might have been deleted, and some vestiges of them are still hanging around. Maybe the first 5 students who took the test did not get either of these questions, and when one of them was served out, the problem began?
I am hoping someone can tell me what this error means. I am assuming that we have a database problem. Is this something we can fix? Is it going to kill the whole instance? Can I simply delete these two questions to resolve the problem? Since I can't find them, which table are they in? Are they in more than one table?
Any advice appreciated.
