How I can add new surveys in the evaluations?
Regarding the custom survey comments, I also think this is a much-needed feature and I suspect it will eventually make its way into the Moodle world.
In the meantime, however, I'm coming to end of my course and I'm wondering if anyone has any suggestions on how to provide students with an online feedback form. I looked into using the quiz and lesson activities for this purpose, but they just won't do. Students need to be able to rate aspects of the course (1 to 10, often/seldom/never, etc.) as well as enter comments. The process should be anonymous and the results should be displayed as an aggregate of all answers. I suppose the only thing to do is to go with an external survey tool.
Has anyone conducted a student survey within Moodle? If not, can anyone recommend a decent program/website for this purpose? Any suggestions would be appreciated.

That's just what I needed ! I've seen Questionnaire hanging around before, but I just assumed it was a modified lesson activity. "Survey" would be the ideal name for this mod, but of course it's already taken. I imagine they will eventaully blend together anyway. Thanks again.
I too am interested in creating surveys. I understand that it isn't ready but thought I would let you know what I am looking for.
I would like to have the survey be self scoring with feed back sent to either the teacher, the student, or both. Basically I want to be able to create a SILL like survey or myers-briggs type survey and have students recieve feed back based on thier scores. I would recieve the results from every student and then be able to aggregate the information to better understand the type of learning that would best suite the majority of students in the class.
Is there any word on when it will be possible to create surveys?
You can create surveys now (it is ready, although it is not in the standard distribution - see my message above)
I personally have used phpsurveyor, which has many features, especially in providing summaries and statistics of information, and linked it to moodle through a webpage resource. However, the phpESP extension mentioned above has been integrated into moodle and only requires a single login, so it may be preferrable.
However, if you want it to provide detailed feedback then the quiz module (or maybe even the lesson module?) is probably the best solution. Perhaps others have comments on this point.
I'm glad the information was useful Fernando. I have been using phpsurveyor and haven't tried the questionnaire module yet, but it is based on phpESP which has a good reputation. It has the advantage of being integrated into moodle, although it could be more tightly integrated perhaps.
With phpsurveyor you had to run the survey outside of the moodle installation or did you include it somehow, perhaps with php includes?
where is this questionaire module? is it an add-in that I have to download extra?
I had to run phpsurveyor outside moodle and link to it in the bottom frame (click "add resource" in the drop-down box and add a webpage link), that is why the phpESP "questionnaire module" adaptation may be a better option, even though I prefer the former a little (very active developer and more functions). The questionnaire module is an add-in that is not even on the modules download page. Get the latest version from CVS at http://cvs.sourceforge.net/viewcvs.py/moodle/contrib/questionnaire/ or download version 4 from the thread at http://moodle.org/mod/forum/discuss.php?d=5639 (though this may not be the latest version).
Note that these options are for collecting information only, and don't allow much immediate feedback to users. If that is required the quiz or survey modules are better options.
