It is better to upgrade. If you don't want to upgrade your entire Moodle, then you should be OK just upgrading just the HotPot module using the link I gave earlier, but if at all possible please upgrade your entire Moodle.
From what I have seen so far, your HotPots work fine on my server, so I think there is something old or odd about your server which is causing some of your HotPots to misfire.
Your problems seem to be confined to "htm" files, so one solution for you would be to use the jcl, jcw, jmt, jmx and jqz files for the time being, until you can schedule an upgrade in a quiet time, such as the upcoming "Golden Week" vacation.
