If the above tool indicates any errors please fix those and try the quiz again on your site.
Otherwise, please could you upload one of the misbehaving Hot Potatoes quizzes (along with any images and sound files that it uses) to my server, and set it up as a HotPot activity in the following Moodle course so we can look at it together:
