I seem to recall that problem. Don't forget to check off the box for each answer or response. Sorry, but that happens to me. My version of 1.6.3 on my homemachine did not duplicate your problem on a multiple choice question. Should not be an issue but did you also hit the refresh on your browser, maybe it is holding the old image.
One of my dumb tricks in 1.5.4 was to copy the HTML code from an area that had a HTML tool to one that did not. For example, I would add a picture to the content area of a multiple choice question, then cut and paste the text HTML code to an answer.
This does not work in 1.6.3 because the Unicode or something has changed the format of the html code. And the HTML edit tool is all over the place now.
However, I think some HTML code can be entered as text and will be recognized in preview or a student view. How to do that needs somebody with more experience. It would be a lot easier to figure out why the HTML editor is not turning on.
