Sometime getting this error "Failed to load PDF document" in my moodle setup. 

Environment: 

Moodle "3.11.6+ (Build: 20220401)" is running on  two servers (APP +DB) without any High Availability. 


1.       Sometime PDF file is not opening in moodle. Gives error on chrome “Failed to load PDF document” & on Edge browser “we can’t open this file,              something went wrong. Refresh.

          Note: FYI, All the data are stored locally on the same application server.

 2.       After refresh PDF Loads properly.

3.       Error was reported in the ngnix error log file when pdf files failed load

[error] 98663#98663: *1049488 open() "/var/www/html/moodle/favicon.ico" failed (2: No such file or directory), client: <IP Address>, server: abc.example.com, request: "GET /favicon.ico HTTP/2.0", host: "abc.example.com", referrer: https://abc.example.com/pluginfile.php/1508/mod_folder/content/0/D1-A10%20-%20example%20TDL%20Environment_ENGLISH.pdf

 4.       Also noticed Some of the PDF file image is not showing/aligned properly as it’s in original file.

5.       FYI, Ghostscript  is properly configured. Tested on different browsers, issue remains same.
