I have resolved the issue. If any one of the "Set protection" boxes are checked for "Print, Modify and/or Copy," the plugin will generate a SECURED/password protected PDF certificate. To avoid this, make sure all boxes are unchecked (see image). When all boxes are unchecked, only then can the PDF certificate be modified, which in my case, is to insert a cover sheet to the PDF.
