Course or activity is not accessible

An admin user 'installed' the attendance plugin, and a teacher user is unable to add it to her course. When she attempts to edit the course and add the activity, after choosing save and return to course or save and display, we receive this error. 

Without debugging turned on, the user gets 'course or activity not accessible.'

With debugging turned on, this becomes

I was unable to find any discrepancy in dir and file permissions. To the best of my knowledge, the course is not hidden, and the attendance activity is set to show in course. Please help.

Quick follow-up: I can login as an admin and add the activity successfully. I can also see it displayed in the course. The teacher does not see it listed in the course, and if she tries to edit and add it, she gets the error from original post.

Have you tested as the teacher? Logged in as them and checked? The error does not look like it is related to Attendance at all. Have you tried clearing caches for the site?
