Hi, I'm writing to report that my team has encountered a similar bug to the above. 8 slots that we set up have seemingly spontaneously disappeared. A team member reported to me that some slots they set up appear to be missing. I then looked at the activity logs and database and was able to assemble a list of 8 slot IDs for which I can see logged events pertaining to their creation, no logged events pertaining to their deletion, and that do not appear in the site database.

We'd like to know if there's any info on whether this is a known issue, what its cause might be, and how we can prevent it happening in the future.

Hello,
I can't really trace what has happened in your installation, but I can confirm that (a) not all operations on slots are logged; (b) unused slots in the past are automatically purged. The latter can be disabled, if you like, by disabling the corresponding task in the "Scheduled tasks" admin view.
