Meanwhile, I agree with Eoin this is very likely a theme or formatting issue. Joseph is on the right track too, I think.
I wonder if they may be related to the "fixes" in Atto added to 3.9 (MDL-45227
) that now insert some required HTML code into the editing boxes? There's been a bunch of squirrely issues with the formatting of this, mostly in the Gradebook, but it could affect Lesson too.
Can you take a look at the html underneath the responses to see if it is either missing that or has some other additional HTML in there that may be coming out into the page when displayed and messing things up?
Meanwhile, it is possible to verify this is a formatting issue and not a issue in the database
by looking at the scores that are set when you create the page and questions. If you have or can install the Ad-hoc database
plugin to run this report, it will show you whether the scores are correct or not. (I suspect they are, but this will verify that). For anyone interested in this, I have posted the SQL

What the output should look like is as in the attached screenshot.
Other things that come to mind not yet asked:
- Is there any css
code at all in the Boost theme settings? Did you bring anything over from Essential you may have customized?
- Did and have you purged the caches and especially the theme cache since the upgrade?
- From your screenshot it looks like this happens to newly added questions as well as previous, upgraded questions, correct?
- The same thing happens in multiple browsers?
- You are using Atto as the editor and Topics as the course format?
