I have just committed a brand new version of the questionnaire to CVS. Changes include:
1) General code cleanup (ongoing).
2) Ability for students to see their responses (configurable).
3) Ability to specify a submission timeframe - always, once, daily, weekly, monthly.
4) New report display for individual responses.
This release is also moving towards full Moodle theme compatability. As such, the 'mystic' theme may not display correctly. If you are using the 'mystic' theme, you will need to change questionnaire using it, or don't install this version yet.

Excellent update!! It fixed my gremlins, namely "Survey does not exist" error with tempates. What's more, sometime in the night it cleaned up all my orphaned templates automatically (the magic of cron). So thank you, and based on other postings here, the update will be appreciated by all!
