Moodle version Moodle 3.9.7+ (Build: 20210520)

Adaptable Theme and accessing quiz results.

When a user wants to grade a quiz they follow this path:

Grades > in grades click a quiz in the header bar > this displays a list of all users that have taken the quiz and the questions they got wrong and right > click on Review Attempt under anyone users name  > this take you to the answers the user selected gave and then a trainer can see the answers and regrade if required. It is this page that is corrupted.

The results are as above. It looks to me as if the CSS has problems displaying. This is true for all browsers (Chrome,Firefox and Edge).

What do we need to correct the problem?

Usual question: What version of Adaptable please? And, give the arrows symbols on the right, what customisations have you done?

Release 3.9.1.2, version 2020073109 on Moodle 3.9.7+ (Build: 20210520).

This was post-upgrade of Moodle from 3.6.x.

I'll have to check the customisations but I suspect the usual logos and colours.
