Form in Moodle for student - marked by teacher

Hi,

I am looking for a way to create a form that the student completed and the teacher marked.

It's a prestart checklist for a 4 Wheel Drive course so it will need to be in Moodle with the other resources.

Does anyone know an activity that will do this?

I use assignment for this. The students have an off-line form, that they save as a PDF. and upload to complete the assignment requirement. The Teacher has to grade the assignment for course completion.
