Is your Moodle hosted on IIS? That's an important detail when asking for help as most Moodle servers are Apache or Nginx.
IIS has a maximum request size which by default is just under 30 MB. Has this been increased?

On my Windows Server 2016 server the steps are:
- Open Internet Information Services (IIS) Manager console.
- Select the website in the left-hand pane and open Configuration Editor:
- In Configuration Editor in Section select the path system.webServer/security/requestFiltering:
- In the list of settings expand requestLimits and change maxAllowedContentLength to the required value (number of bytes, 134217728 = 128 MB):
This setting can be applied to the IIS server and/or to websites individually.
