To see a users cohorts - I use the Contributed Reports plugin and the extremely helpful SQL script that Randy Thornton provided -

To remove a cohort - Course Admin>Enrolment Methods
All of the current cohorts will be displayed - delete or hide the cohort that should no longer be used.
